Oral surgery can feel overwhelming, but proper preparation and aftercare make all the difference in your healing experience. Whether you’re facing wisdom tooth extraction, dental implant placement, or other oral surgical procedures, understanding what to expect before and after surgery helps ensure optimal results and minimal discomfort.
Pre-Operative Preparation: Setting Yourself Up for Success
Make sure your surgeon knows what medications you take, as some can negatively interact with anesthesia medications. This includes prescription medications, over-the-counter drugs, and supplements. Certain diabetes medications and blood thinner medications may need to be withheld prior to surgery. Your oral surgeon will have discussed this with you.
Do not eat or drink anything for 8 hours prior to your sedation or general anesthesia. For morning surgery, no food or liquids after midnight the night before surgery. However, if you need to take medications, drink no more than half a glass of water.
Your clothing choices matter on surgery day. Please wear loose-fitting clothing with sleeves that can be rolled up past the elbow and low-heeled shoes. Anesthesia monitoring requires access to your arms and upper chest, making a T-shirt a good choice. Contact lenses, jewelry, and dentures must be removed before surgery. Do not wear lipstick, excessive makeup, or nail polish on the day of surgery.
You will need someone to drive you to and from your appointment. The patient should not drive a vehicle, operate any machinery, or make important decisions for 24 hours following intravenous anesthesia. A responsible adult escort must be present at the time of your surgery, be able to stay throughout your surgery, and be able to safely take you home to make sure you are recovering well after the operation is over.
What to Expect During Recovery
The first 24 hours after surgery are crucial for proper healing. Bite down firmly but gently on the gauze packs that have been placed over the surgical area(s). Make sure that the gauze remains in place, undisturbed for 30 minutes, then reapply with clean gauze. Remove the gauze pack(s) after one hour. Do not rinse, spit, use straws, smoke, or brush your teeth for the first 24 hours!
Managing swelling is essential for comfort. Swelling from oral surgery is normal and reaches its maximum in 48 hours. Swelling can also be minimized by applying cold compresses to your face every hour (30 minutes on, 30 minutes off) for the first 48 hours. For the first 24 hours, apply ice to your jaw for 30 minutes, remove it for 15 minutes, then do it again. If you had surgery on both sides of your mouth, switch from side to side with a single ice pack.
Post-Operative Care Guidelines
Pain management requires a proactive approach. Some form of pain reliever should be taken before the numbness goes away. Over-the-counter medications like ibuprofen (Advil® or Motrin®), acetaminophen (Tylenol®), or aspirin are adequate if there are no allergies, they have been tolerated in the past, and they do not interfere with any other medications. On the day of your surgery, you may take 2-3 ibuprofen pills (400-600mg) prior to coming to the office. If you are being sedated, you should take the medication with only enough water (a small sip) to swallow the pills. Having the medication before your operation can significantly reduce both post-operative pain and swelling.
Proper oral hygiene supports healing. Avoid rinsing the mouth for 24 hours after surgery. The following day, you may begin gently rinsing with warm (not hot) salt water. Dilute 1 teaspoon of salt in 8 ounces of warm water and rinse gently 3 or 4 times a day for 1 week. You may start normal tooth brushing the day after the surgery or after bleeding is controlled. Be careful to avoid the surgical site when brushing your teeth.
Your diet plays a crucial role in recovery. A liquid or non-chew diet is recommended the day of surgery, and then soft foods high in vitamins and protein is recommended for the next several days. Increase your fluid intake. Gradually resume your normal diet as soon as possible. Avoid any foods that contain nuts, seeds, popcorn, rice, corn, or chips. All food and drink should be consumed chilled/cool until the numbness wears off. Do not use a straw for 1 week as this may disturb the blood clot and/or promote bleeding.
Important Restrictions and Warning Signs
Avoid smoking or chewing tobacco for 1 week after surgery. Alcohol should not be consumed while taking pain medication and/or antibiotics. Avoid strenuous activity and do not exercise for at least 3–4 days after surgery.
Watch for signs that require immediate attention. Signs of infection include increased pain and swelling after 3 days, redness, warmth, drainage of pus, foul smell, and fever. If these occur, contact our office. Look for infection, fever, swelling that lasts more than 2 days, and trouble swallowing or breathing. Call your dentist right away if any of these things happen to you.
